In this Elden Ring guide, I will show you how to do Millicent’s quest – a massive, complex quest yet so much fun. Millicent’s quest is a fairly late-game quest and does not fall into the main storyline. Hence, it is possible to miss it.
You can start Millicent’s questline by talking to an NPC called Gowry in Gowry’s Shack, Caelid. He will ask you to find a Golden Needle to give to Millicent, who is plagued by scarlet rot. You can acquire this item by fighting a boss called Commander O’Neil in the middle of a swamp filled with scarlet rot.
Yes, that’s how the quest begins.😨
Millicent’s quest is one of the most challenging and multi-layered quests in Elden Ring because it is spread across the Altus Plateau, Caelid, Miquella’s Haligtree, and a few more locations.
At the end of the questline, you must make a tough choice of assisting or killing Millicent. This will determine the outcome of the quest and also your rewards.
So, Millicent’s questline is for the truly dedicated Elden Ring players only, and I am one of them, are you?

Meet Gowry at Gowry’s Shack (Caelid)
Gowry is an NPC in Elden Ring who resides at Gowry’s Shack in Caelid. He was once a great sage in his final days of life and wishes to cure the young Millicent from a rotting sickness.
You can kill Gowry right here to acquire Flock’s Canvas Talisman and Gowry’s Bell Bearing, but I recommend you do not do that. You cannot do Millicent’s quest without Gowry, so keep him alive.
When you speak to Gowry, he will ask you to find a Golden Needle.
This is Gowry’s Shack location in eastern Caelid, just before Sellia, Town of Sorcery. You can reach this location by following the Caelid Highway to the east and the north.
Fight Commander O’Neil in Inner Aeonia Swamp
From Gowry’s Shack, head west to the Inner Aeonia Swamp to defeat Commander O’ Neil and collect Unalloyed Golden Needle.
The biggest threat while fighting Commander O’Neil is the NPCs he summons before the fight begins. I recommend killing off these NPCs before you go all in for the Commander; otherwise, they will just hinder your fight.
After defeating Commander O’Neil, you get the Unalloyed Gold Needle and Commander’s Standard as a reward.
Give Gowry the Unalloyed Gold Needle for Repairing
After defeating Commander O’Neil, return to Gowry at Gowry’s Shack and talk to him. Hand him the Unalloyed Gold Needle.
As a reward for retrieving the Golden Needle, Gowry will gift you with Sellia’s Secret – a document that tells of the three towers in Sellia, Town of Sorcery, that unlocks the Sellia Backstreets Site of Grace.
Sellia, Town of Sorcery, is also a location for one of the Imbued Sword Keys. So, you must have probably already lit these towers and unlocked the Sellia Backstreets area. If not, this is your chance to do so. 😃
Before heading to Millicent, you need to collect the repaired Unalloyed Gold Needle from Gowry. So, quit the game and reload it. Meet Gowry again and collect the repaired Unalloyed Gold Needle.
Note🔔: It is important that you let Gowry repair the needle and collect it from him because only the repaired needle can stop the scarlet rot that’s killing Millicent.
Meet Millicent at Church of the Plague (Millicent’s Location #1)
Head southeast from Sellia’s Backstreet Site of Grace to reach Church of the Plague. You can also reach Church of the Plague from Fort Faroth, head southwest, and take a Spiritspring to get down on the terrains.
You will find two guards at the entrance if this is your first time at Church of the Plague. Or else, do what I did, without inviting their aggro, just enter the insides of the church from the back.
You will find Millicent lying near the right wall. Initiate a dialogue with her.
When the dialogue exhausts, hand the Unalloyed Gold Needle to Millicent. She will know what to do with it. You, after exhausting Millicent’s dialogue, should sit at the nearest Site of Grace.
After stabbing herself with the needle, Millicent will regain her strength as the effects of scarlet rot subside. This will only happen after you rest at the nearest Site of Grace.
As a reward for curing her scarlet rot infection, Millicent will reward you with a Prosthesis-Wearer Heirloom – a talisman that raises Dexterity by 5.

Now, rest at the nearest Site of Grace again, and Millicent will move to Gowry’s Shack.
Meet Millicent at Gowry’s Shack (Millicent’s Location #2)
Millicent has now moved to Gowry’s Shack, and if you wish to continue Millicent’s questline, you have to meet her at the Shack. When you see her, initiate a dialogue.
After exhausting Millicent’s dialogue, quit the game and reload it. Now, Millicent will be gone, and Gowry will reappear. Now, you can study sorcery from Gowry.
You can buy any or all of the three sorceries from Gowry – Glintstone Stars, Night Shard, or Night Maiden’s Mist. I really suggest you buy the last one if you like to incorporate magic into your playing style.
Collect Valkyrie’s Prosthesis for Millicent (Millicent’s Location #3)
Head to Altus Plateau and locate the Erdtree-Gazing Hill Site of Grace. This is where you’ll find Millicent now.
You will find Millicent waiting for you on the top of a cliff. She is just north of the Erdtree-Gazing Site of Grace. She will hint at not having her sword arm. If she had, she could assist you in battles.
Now, head to The Shaded Castle, located in the northernmost region of the Altus Plateau. This castle has a lot of items to loot if you decide to take your time and stroll around the castle area.
However, I must warn you that the region is filled with poisonous waters that can instantly add the Poison status effect on you. So, proceed with care.
You’ll see a Cleanrot Knight guarding an important chest inside the castle, defeat him and move inside the castle.
Inside the chest, you will find Valkyrie’s Prosthesis. This item is required to advance in Millicent’s questline.
After collecting the Valkyrie’s Prosthesis, return to Millicent. She will still be standing at the previous location – Erdtree-Grazing Hill Site of Grace. Give her the Valkyrie’s Prosthesis, and she will thank you for it.
Millicent will offer to help you in your future battles if the arm serves her correctly, which it does. She will also mention a hint about Malenia’s hidden location in the north.

Go to Domimula, Windmill Village, Altus Plateau (Millicent’s Location #4)
Now, head to Domimula, Windmill Village, in the northern region of the Altus Plateau. Despite the jolly portrayal of the Celebrants, this place holds a sinister secret.
When you reach the Windmill Village, you must fight a Godskin Apostle and rest at the Windmill Heights Site of Grace. Only after this will Millicent appear near the Site of Grace.
During the conversation, Millicent will offer to be summoned in your future battles to assist you in fights. After exhausting her dialogue, rest at the Site of Grace, and she will move to her next location.
Meet Millicent at Ancient Snow Valley Ruins (Millicent’s Location#5)
After your encounter with Millicent at Windmill Heights, she will move to Ancient Snow Valley Ruins in the Mountaintops of the Giants region. You can also find a Legend Boss called Fire Giant in this region. You can still do Millicent’s quest after killing Fire Giant.
Meet Millicent near the Ancient Snow Valley Ruins Site of Grace and converse with her. She will reveal many details about Malenia and her location – the commander with the medal, the hidden location of the Haligtree, and more.
I have covered how to reach the Haligtree and fight Malenia, the Blade of Miquella. According to many Elden Ring players, she is considered the toughest boss ever in FromSoftware games.
Go to Haligree and Find Millicent (Millicent’s Location #6)
After solving the puzzle in the Ordina, Liturgical Town, head to the Haligtree location. In this location, you will find Millicent at the Prayer Room Site of Grace at Elphael, Brace of the Haligtree.
Millicent will not move from this spot until you have killed the nearby Lesser Ulcerated Tree Spirit mini-boss. To reach the mini-boss, travel via the Drainage Channel Site of Grace and exit the doorway. Go west and climb up the ladder at the end of the corridor.
In this same conversation, Millicent hints at her relationship with Malenia. She could be Malenia’s daughter, sister, or just an offshoot. Millicent and her sisters are an offshoot (buds) of Scarlet Aeonia (flower), which is essentially Malenia. So, you can consider Millicent and her sisters as daughters of Malenia or offshoots of her.
Challenge or Assist Millicent?
After defeating the mini-boss, two summoning signs will appear nearby. One is a red one that lets you challenge Millicent for a duel. The other is a white one that lets you assist Millicent in killing her four sisters.
Let’s explore both endings.
Millicent’s Quest Ending #1 (Challenge Millicent)
I changed my armor and weapon before getting into this fight. Although Millicent is not much of a challenging opponent, it’s best to wear armor with high poise and wield one of the best katanas in Elden Ring, like Rivers of Blood or a Moonveil.
Go near the red sign and be summoned to challenge Millicent.
After you challenge Millicent and defeat her successfully, you will receive a Furlcalling Finger Remedy and a Rune Arc.
When you return to your world, you will receive another reward – Millicent’s Prosthesis. This talisman boosts Dexterity by 5 and increases attack power with successive attacks.
Millicent’s Quest Ending #2 (Assist Millicent)
If you want to assist Millicent, stand in front of the while summoning sign, be summoned to her world, and aid her in killing her sisters.
Millicent’s sisters will attack her altogether. You need to get their attention and invite them to fight with you.
Once you help Millicent defeat her sisters, you will be rewarded with Rotten Winged Sacred Insignia, a talisman that greatly raises attack power with successive attacks.
Collect Unalloyed Gold Needle from Millicent
If you choose ending #2, i.e., assist Millicent, you will be rewarded with a talisman that more or less does the same thing as the talisman you get from betraying her.
You can only choose one of the two endings to Millicent’s quest, and I strongly suggest you choose the second one – assist Millicent.
After the fight, Millicent lies near the summoning signs, taking her last breaths. Be careful here since she is very fragile. If you accidentally step on her or walk over her, she will die, and you won’t be able to revive her.
When you converse with her, she will tell you that she has removed the Unalloyed Gold Needle from her chest and wishes to die in peace.
After exhausting Millicent’s dialogues, reload the game and return to the same spot. You will find Millicent dead, and you can collect the Unalloyed Gold Needle off her corpse.
Exchange Needles at Malenia’s Boss Arena
If you have defeated Malenia, then amazing!
If you haven’t, good luck with it. 😂
After defeating Malenia, you can go to her arena. You will see that she has bloomed into a scarlet flower. Go near the flower and exchange the Unalloyed Gold Needle with Miquella’s Needle. You will also get Somber Ancient Dragon Smithing Stone.
Miquella’s Needle is a consumable item that subdues the Flame of Frenzy if inherited. It allows you to cheat fate and avoid becoming Lord of Frenzied Flame.
Miquella’s Needle is used at Dragonlord Placidusax’s arena at Crumbling Farum Azula to reverse the Three Fingers’ flame of frenzy. This is the only way to undo the Lord of Frenzied Flame ending.
To use the needle, simply select it from your inventory anywhere in the arena and use it. You do not need to fight the Dragonlord Placidusax. Just stand in the arena and use the needle.
Return to Gowry & Collect Talismans
This is not necessary, but if you want to collect two powerful items, do it. The choice is yours.
Return to Gowry at Gowry’s Shack and kill him. You will get Flock’s Canvas Talisman – it raises the potency of Incantations by 8%. You also get Gowry’s Bell Bearing, which can be offered to the Twin Maiden Husks to gain access to new shop items.
Final Thoughts
Even though Millicent’s questline is missable and not mandatory, it is definitely worth it. The rewards are absolutely worth all the hard work you do for Millicent, and the quest connects you to some of the most important characters in Elden Ring.
You can do Millicent’s quest anytime you want, even after killing the final boss. So, do it at your leisure.
Also, I strongly recommend you choose the “Assist Millicent” ending as that will give you the best rewards and peaceful death for Millicent, something she truly deserves.
